| Home Health care is not an option for everyone. Generally, the person receiving care or treatment at home must be well enough to be cared for in the home setting. |
Home Health Services are provided by nurses, therapists, medical social workers and home health aides depending on the services required. The services include Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Work and Home Health Aide. Services are determined by the appropriate needs of the individual and will be planned, coordinated and made available under the direction of the attending physician and qualified staff.

Hospice
Hospice is a team approach to the care that is focused on the alleviation of the physical symptoms and the provision of emotional, psychosocial and spiritual support to patients and their families or significant others. Hospice care is palliative, not curative in its goal or treatment for patients with a terminal illness. The hospice team includes a medical director, nurses, medical social services, home health aides, homemakesr, spiritual care services, dietary counseling, volunteer services and bereavement counseling services as appropriate to the patient's care.
Normal office hours are 8 a.m. - 4:30 p.m. A hospice and home health nurse are available 24 hours a day, 7 days a week. For more information, please call 570-416-0561.
