The registrar will need the physician order, insurance cards and referral/precertification where applicable.

Pre-registration hours: Monday through Friday,  11:30 a.m. to 4:30 p.m.

Outpatient Scheduling: Monday through Friday,  11:30 a.m. to 4:30 p.m.
